JUDGMENT NOTE
May ~ 2002
On demand, I, MARY HUNTER, the undersigned, promise to pay to the order of TODD
BAIRD LINDSEY FOUNDATION, the sum of $3,487.00 Dollars, without defalcation, for value
received, without interest thereon, until satisfied in full.
The undersigned hereby empowers any prothonotary, clerk or attorney to any court of record
within the United States or elsewhere to appear for the undersigned and, with or without declaration,
to confess judgment at any time or times against each, any or all of the undersigned and in favor of
the holder hereof for the above sum plus interest thereon from the date hereof, with costs of suit or
attorneys' commission of fifteen percent for collection. The undersigned hereby releases all errors
and expressly waives all rights to any stay of execution, sequestration of rents and exemption of any
property from levy and sale of execution under any law or rule of court now in force or hereafter
enacted. All of the foregoing promises are the joint and several promises of the undersigned and shall
bind the undersigned, heirs, personal representatives, successors and assigns. The undersigned and
all endorsers waive protest, demand and notice of nonpayment of this note.

DISCLOSURE FOR CONFESSION OF JUDGMENT
I acknowledge that I have executed a Judgment Note this,~D~ day of May, 2002, in the
amount of $3,487.00 Dollars obligating me to repay that amount, until satisfied, to TODD BAIRD
LINDSEY FOUNDATION, or its successors and assigns.
I understand that the Note contains wording that would permit TODD BAIRD LINDSEY
FOUNDATION to enter judgment against me at the Courthouse, whether or not said Note is in
default, without notice to me and without offering me an opportunity to defend against the entry of
judgment, and that the jl~tgment may be collected by any legal means.
In executing the Note, I am knowingly, understandably, and voluntarily waiving my rights to
resist the entry of judgment against me at the Courthouse, and am consenting to the confession of
judgment.
I certify that my annual income exceeds Ten Thousand Dollars ($10,000.00) and that I
received a copy of this Disclosure at the time of signing.
I certify that the Note is not given to secure a consumer loan.
I certify that I have been informed I may make lump sum payments to satisfy the outstanding
balance at any time, without penalty.
