PETITION FOR NAME CHANGE
1. The Petitioner in this matter is Lynda A. Williams, who is an adult individual
who currently resides at 1517 Inverness Drive, Mechanicsburg, Cumberland County,
Pennsylvania 17050.
2. The Petitioner has resided at her current residence, 1517 Inverness Drive,
Mechanicsburg, Cumberland County, Pennsylvania 17050, for two and one half years
immediately prior to the filing of the Petition for Name Change, and prior to that, at 4154
Nantucket Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050 for seven
years. The Petitioner wishes to change her name formally to Lynda A. Bruno.
4. If this Petition is approved, the Petitioner intends to use the name Lynda A.
Bruno consistently, non-fraudulently, and exclusively.
5. It is not the purpose of this Petition to avoid any taxes or any other debts.
