SHARMA, Petitioner IN THE COURT OF COMMON PLEAS CUMBERLAND COUNTY, PENNSYLVANIA PETITION FOR CHANGE OF NAME This Petition for Change of Name is submitted pursuant to the requirements of Statute 54 P.S. 9701, et seq. In support of this Petition, Petitioner avers as follows: 1. Petitioner is Sushil C. Sharma, an adult individual residing at 3 Thyme Court, Mechanicsburg, PA. During the past five years, Petitioner has lived at that address, and at 915 Scottish Court, Mechanicsburg, PA. 2. Petitioner is employed by NRA Group, LLC, d/b/a National Recovery Agency, at 2491 Paxton Street, Harrisburg, Pennsylvania 17111, as its director of operations. Petitioner has been employed with National Recovery Agency for three years. 3. Petitioner was born in India, immigrated to the United States with his family, received his post-high school and post-graduate education in the United States. He has been employed in the United States in various positions of responsibility for many years. 4. Through usage and as an accommodation to English-speaking persons, Petitioner's first name has become anglicized as "Shell," rather than "Sushi!." He is now .. routinely and commonly referred to as Shell Sharma, and his given name is confusing to business colleagues and with persons with whom he routinely interacts. 5. Petitioner is generally known and identified by the name "Shell." Petitioner desires to adopt as his legal name: Shell Sushill Sharma 6. Petitioner requests the Court to consider this request for Court-approved name change and, after hearing, to grant the relief requested. 7. Pursuant to 54 Pa. C.S.A. ~ 702(b), in all such petitions, the Court is required to obtain a report from the Pennsylvania State Police. That report, when obtained will reflect that Petitioner was subject to certain proceedings in the Federal Court System, docketed to 96-CR-321-03. 8. Those proceedings resulted in a judgment of conviction related to business ownership and not by virtue of any employment positions he has held. The sentence was fully carried out, and all obligations of parole and satisfaction of sentence have been duly met, more than five years ago. 9. No confusion or uncertainty will be created by this change of name, and the name change, if approved, will conform Petitioner's legal name with the custom and usage as persons address him. 10. The filing of this petition and the scheduling of a hearing on the matter will be published in accordance of the statute 54 P.S. ~ 701(a.1 )(3)(ii). 2 ....
