PETITION FOR NAME CHANGE
Jeffrey A. Graf and Bonita F. Graf, by their attorneys Broujos & Gilroy, P.c., set forth
the following in accordance with 54 Pa.C.S.A. ~701 et seq:
1. Petitioners are Jeffrey A. Graf and Bonita F. Graf and are husband and wife and adult
individuals residing at 671 Deer Road, Boiling Springs, Cumberland County,
Pennsylvania 17007.
2. Petitioners file this Petition for Name Change on behalf of their minor daughter, Flor
De Maria Graf, born September 1, 1990.
3. Petitioners adopted Flor De Maria Graf in Guatemala in 1991; the minor child was in
the custody of the Petitioners since September, 1990 and has remained in the custody
of Petitioners since that time.
4. The Petitioners and the minor child, Flor De Maria Graf, have resided at 671 Deer
Road, Boiling Springs, Cumberland County, Pennsylvania, for at least five years prior
to the filing of this Petition.
5. Petitioners and the minor child desire that the minor child's name be changed to
Andrea Desiree Graf.
6. The reason for this name change request is that the minor child has been going by the
name of Andrea Desiree Graf and that name appears on some of her school records at
the Cumberland Valley School District, on her baptismal records and other official
record.
WHEREFORE, the Petitioners request your Honorable Court to set a hearing for
purposes of taking testimony on this name change petition.

ORDER OF COURT
AND NOW, this 30th day of March, 2007, after hearing
and review of the Petitioner's exhibits, the Court does find that
the requirements outlined in Title 54, Section 701, et. seq. for
judicial change of name have been satisfied.
IT IS HEREBY ORDERED AND DIRECTED that the name of the
child F10r DeMaria Graf shall be changed from this date forward
to Andrea Desiree Graf.
