1. Applicant is Desiree Ebo Sekye-Aldoo who resides at 906 Cocklin Street, Mechanicsburg,
PA 17055.
2. Previously and for the past 5 years Applicant resided at:
i) From November 24, 2006 until the present, Applicant has lived at 906
Cocklin Street, Mechanicsburg, PA 17055.
ii) From February 2005 until November 24, 2006, Applicant lived at 354
Stonehedge Lane, Mechanicsburg, P A.
iii) From April 2003 until February 2005, Applicant lived at 5340 Oxford Circle,
Apt. 49, Mechanicsburg, PA.
iv) From August 2001 until April 2003, Applicant resided at 506 South 13th
Street, Oakland 346, Indiana, P A.
3. It is not anticipated that any judgments have been entered against the Applicant in any
County where he has resided for the past 5 years. At the hearing set in this matter Applicant
intends to admit into evidence proof of the same from Indiana and Cumberland Counties.
4. It is not anticipated that the Applicant has a criminal conviction on his record and evidence
of the same will be presented at the hearing to be held in this matter.
5. Desiree Ebo Sekye-Aldoo never used the last name Sekye-Aldoo.
6. Applicant's last name was incorrectly spelled on his birth certificate as Sekye-Aidoo. A
copy of the birth certificate is attached hereto as Exhibit "A".

7. The correct spelling of Applicant's last name is Sekyi-Aidoo and that has always been the
last name used by Applicant.
8. Until the time he was about 6 years old, Applicant used the first name of Desiree.
9. When he was born, Applicant's parents wanted to give Applicant an English name.
10. Around the time Applicant was 6 years old, Applicant's parents realized they mistakenly
gave Applicant, who is male, a female first name.
11. After Applicant's parents realized their error in giving their son a female name and up until
the present time, Applicant's parents and generally everyone else began referring to
Applicant as Derrick.
12. Attached as Exhibit "B" is the supplement which Applicant filed with United States
Citizenship and Immigration Service (USCIS) on or around March 20, 2006. This
supplement basically represented to USCIS the same facts as this application regarding his
first name (Desiree) and the spelling of his last name (Sekye-Aidoo instead ofSekyi-Aidoo).
13. Applicant was granted U.S. permanent residency (his "green card") by USCIS, however, the
USCIS officer who interviewed Applicant took the position that he (the officer) did not have
the authority to change Applicant's name on the green card from that which appeared on
Applicant's birth certificate.
14. Attached as Exhibit "C" is the front of Applicant's green card.
15. The name which appears on that card is Desiree Ebo Sekye-Aldoo.
16. On the green card, it appears evident that a mistake was made regarding Applicant's last
name. On the green card, Applicant's last name is spelled "Aldoo" instead of"Aidoo".
17. Applicant never heard of his last name being spelled "Aldoo" until he received his green
card in the mail.

18. Therefore, absent a formal name change (the object of filing this application), Applicant will
officially be known, at least by US CIS, as Desiree Ebo Sekye-Aldoo.
19. The 3 changes to the name Desiree Ebo Sekye-Aldoo being requested are:
i) Desiree to Derrick;
ii) Sekye to Sekyi; and
iii) Aldoo to Aidoo.
20. Applicant files this application for purposes of having his official name be the same as the
name he has used since he was 6 years old.
WHEREFORE, Desiree Ebo Sekye-Aldoo requests the Court issue an Order directing
that his legal names be changed to Derrick Ebo Sekyi-Aidoo.

AND THE G-325A OF DERRICK E. SEKYI-AIDOO
The Christian name on my birth certificate is incorrect. That name is Desiree Ebo Sekye-
Aidoo. My mother never intended to name me Desiree. The people who completed the birth
certificate made that mistake from the beginning. After the name Desiree appeared on my birth
certificate, however, I used that name until I was about 6 years old and in the first grade. Around
that time, my parents were told that Desiree was a female name. From that point forward I used
the first name Derrick.
My last name has always been Sekyi-Aidoo and not Sekye-Aidoo. It was misspelled on
my birth certificate.
I certify, under penalty of perjury under the laws of the United States of America, that the
foregoing is true and correct. Furthermore, I authorize the release of any information from my
records which U.S.c.I.S. needs to determine eligibility for the benefit that I am seeking.
