PERFORMED AND MATERIALS FURNISHED
To The Prothonotary of Cumberland County:
Claimant, James R. Myers, Jr., of the Township of South Middleon, Carlisle,
Pennsylvania, carrying on a business at 40 Spring Garden Estates, in the said Township
of South Middleton, files his claim for the sum of Five hundred twenty-five Dollars and
00/100 ($525.00) together with interest and costs against Niles C. Logue and his wife
Kathleen M. Logue, and against all that certain dwelling at 614 North Hanover Street in
the Borough of Carlisle (Parcel #06-20-1800-031), for certain materials furnished and
labor performed in doing the remodelling work in and about the said dwelling and makes
the following statement of his demand:
The name of the party claimant is James R. Myers, Jr., and he files this
claim as contractor for purchase and installation of a new shower stall.
The name of the owner of said property at the time of the fumishing of
said material and labor and the attaching of the lien therefore is the
aforesaid Niles C. Logue and Kathleen M. Logue, and their address is 614
North Hanover Street, Carlisle, Pennsylvania, 17013, and their mailing
address is 61 Broad Reach, Unit M-37, Waymough, MA 02191-2278.
The date of the completion of the claimant's work was May 6, 2002.
The said claimant therefore claims to have a lien upon the said dwelling as
hereinbefore described for the mount of the claim, the balance now due, the said sum of
$525.00, from the said 6th day of May, 2002, according to the acts of assembly in such
cases and made and provided.
