Whenever used in this Order, the following terms are detined
as tollows:
(a) "Participant" means Ronald R. Shuey, whose social security
number is 189-30-4454, whose last known address is 314 Berkshire
Road, Mechanicsburg, Pennsylvania, 17055, whose date of birth is
April 20, 1937, and who is owner of the Plan, hereinatter def.ined.
(b) "Alternate [layee" means Thelma Joyce Shuey, whose social
security number is 204-30-8167, whose last known address is 520
Reno Street, Apartment 3, New Cumberland, Pennsylvania, 17070,
whose date of birth is October 1, 1940, and who is the tormer
spouse of the Participant having been married to the Participant on
December 16, 1961, and having been divorced from Participant on
March 13, 1995.
(c) "Plan" means the Union Central Lite Insurance company
Annuity Policy Number C22015107 and any successor plans.
(d) "Code" means the Internal Revenue Code ot 1986, as
amended.
(e) "ERISA"means the Employee Retirement Income security Act
of 1974, as amended.
JURISDICTION OF THE COURT
This Order is entered pursuant to the laws of the Commonwealth
of Pennsylvania, which has jurisdiction over the equitable
distribution of the marital property between the spouses herein in
the above-referenced action for dissolution of the parties' marriage
pursuant to 23 pa C.B.A. 3501, et seq.
IT IS HEREBY ORDERED AND ADJUDGED as follows:
(a) Amount to be Paid to Alternate Payee.
The Alternate
lIayee is awarded the sum of $9,586.57 from Annuity Policy Number
C22015107.
(b) F;equency of PaymElnts. Alternate Payee is to receive
the amount in paraqraph (a) above in one lump slim. Thllre shall be
no additional payments from the Plan to Alternate Payee.
(c) Taxes. The Aiternate Payee shall be responsible for
any and all taxes payable by reason of any of the payment awarded
to Altern~te Payee hereunder.
ORDER TO BE CONSISTENT WITH PLAN
Nothing contained in this Order shall be construed to require
the Plan or its Plan Administrator to:
(a) Provide to the Alternate Payee any type or form of
benefit or any option not otherwise available to the Participant
under the Plan; or
(b) Provide

EXECUTION OF THIS ORDER
Copies of this JUdgment and Order shall be served by the
attorney for the Alternate Payee upon the Plan Administrator, who

of the recllipt of a copy of this Order by the Plan Administrator
and of the Plan's procedure for determining the qualified status of
the Domestic Relations Order; and
(b) Within a reasonable period of time after receipt of a
copy of the Judgment and Orde~, determine whother this order is a
QDRO, and notify the Court, the Participant and the Alternate Payee
of such determination.
INTENT OF 'rHE PARTIES AND OF THE COURT
This Order and these provisions are intended to be a ODRO made
pursuant to and in accordance with Section 206(d)(3) of ERISA, and
its provisions shall be administered and interpreted in conformity
with that law.
CONTINUING JURISDICTION OF THE COURT
The Court retains jurisdiction over this Order to amend as
necessary to establish or maintain its qualification as a QDRO
under ERISA.
