THEVENIN PETITION FOR SETTLEMENT OF A SMALL ESTATE The petition of Mary Ellen Woodside, Executrix under the Codicil, dated AUgUBt 28, 1970, to the Last Will and Testament of the decedent, Ellen A. Thevenin, respectfully represents I 1. The decedent, late of the Borough of Carlisle, Cumberland County, pennsylvania, died testate on September 13, 1993. A copy of the death certificate is attached as Exhibit A. 2. At the time of decedent's death,her only dsset was a bank aocount with First Fidelity Bank, N.A., 550 Broad St., Newark, N.J. 07102, with a balance of $3,027.06, as is shown on the attached bank statement marked Exhibit B. 3. Expenses incurred due to the death of Ellen A. Thevenin were as follows I Faykosh Funeral Home Belvedere Medical Center Sarah Todd Mem. Home Mary Ellen Woodside, reimburse for medical bill Douglas,Douglas & Douglas Register of Wills, filing fee Total $1,759.50 11.88 575.50 60.00 100.00 17.00 $2,523.88 4. After payment of the above bills, there is a balanoe of $503.18 in the aforesaid checking account. S. Pennsylvania Inheritance Tax in the amount of $30.20 has been paid on the aforesaid $503.18, leaving a balanoe in the . aforesaid checkJ.ng account of $472.98. 6. In accordance with the second Codicil, dated April 1, 1986, to the Last Will and Testament of Ellen A. Thevenin,all three of which are attached hereto marked Exhibit C, tho remainder of her estate was to be given to the propagation of the Faith, Roman Catholic Churoh. " WHEREFORE, it is prayed that your petitioners be permitted to make distribution to the Propaqation of the Faith, Roman Catholic Church as set forth in paraqraph 6 of this petition.
