bank with a place of business located at 219 Main Street, Tilton,

2. The Defendant is David P.
place of residence located at
Mechanicsburg, PA 17055.
3, At the written request of the Defendant, Plaintiff issued
to Defendant a reusable personal loan account that allowed
Defendant to aCCE:SIB available cash by writing checks, under
account number 4310-391S-3570-0998 and, at all times relevant
hereto, Defendant was the holder of said loan account issued
through Plaintiff's credit facIlities. A true and correct copy of
Defendant's application is attached hereto, made a part hereof and
marked Exhibit "A",
