2, The Defendant, CHERIE L. RENAUT, is an adult
individual residing at 832 Factory Street, Carlisle,
Cumberland county, Pennsylvania,

AFFIDAVIT OF CONSENT TO DIVORCE
AND NON-PARTICIPATION IN COUNSELING
1. A Complaint in Divorce under Section 3301(c) of
t.he Divorce Code was fill':d on f'ebruary 9, 1994.
2. Defendant acknowledges that service of the Com-
plaint was made by certified mall on February 17, 1994.
3. 'l'he marriage of Plaintiff and Defendant is ir-
retrievably broken and ninety (90) days have elapsed from
the date of filing the Complaint.
4. I have been advised of tl\e availabllity of mar-
riage counseling and understand that I may request that the
Court require that my spouso and 1 participate in counsel-
ing. Being so advised, 1 do not request that the Court
require counseling prior to a Decree in Divorce being handed
down by the Court,
5. 1 consent to the entry of a final decree of
divorce,
6. 1 understand that if a claim for alimony, alimony
pendente lite, marital property or counsel fees or expenses
has not been filed with the Court before the entry of a
final Decree in Divorce, the right to claim any of them will
be lost.
I verify that tho statements made in this Affidavit are
true and correct. I understand that false statements herein
are made subject to the renalti.es of 18 Pa. C, S. S4904
relating to unsworn falsification to authorities.
