AND NOW, this 14th day of February, 1994, oomes Petitioner,
Denise F. Newhouse, by and through her attorney, Michael J.
Hanft, Esquire, and files the following Petition, and in support
thereof, avers the followingl
1. Petitioner, Denise F. Newhouse, is an adult individual
residing at 9 Bellaire Avenue, Carlisle, cumberland county,
Pennsylvania.
2. Petitioner is the natural mother of Aaron Robert
Newhouse, born January 5, 1994 in Carlisle, Cumberland county,
Pennsylvania.
3. Petitioner's maiden name was Denise F. Deibler.
4. Petitioner has been a resident of Cumberland county,
Pennsylvania for at least six (6) months previous to the filing
of this Petition.
5. Petitioner desires to change the name of her son from
Aaron Robert Newhouse to Aaron Robert Deibler.
6. Petitioner dOBS not wish to change the name of her son,
Aaron Robert Newhouse, for any illegal purpose or to avoid any
obl igation.

7, The natural father of Aaron Robert Newhouee is Todd W.
Newhouse, who is Petitioner'~ husband.
8. petitioner is estranged from her husband, Todd W.
Newhouse, and has filed for divorce in the Court of Common Pleae
of Cumberland county, Pennsylvania at docket number 653 civil
1994.
9. Todd W. Newhouse is an adult individual residing at 550
Second Street, carlisle, Cumberland County, Pennsylvania.
lQ. Todd W. Newhouse consents to tne proposed name change
of Aaron Robert Newhouse. Attached hereto as Exhibit "A" is a
consent signed by Todd W. Newhouse.
11. Aaron Robert Newhouse does not have a Social Security
Number.
WHEREFORE, Petitioner, Denise F. Newhouse, respectfully
requests this Honorable Co~rt to issue an Order changing her
son's name from Aaror Robert Newhouse to Aaron Robert Deibler.
