AND NOW, oome. Petitioner, cindy L. Boonie, by and through her
attorneYI, MANCRE AND WAGNER, who make. the following averment. in
lupport of thie License Suspen.ion Appeal.
1. Petitioner, cindy L. Boonie, an adult individual, is a
licen.ed driver within the Commonwealth ot Penneylvania with a
reddence address of 17 Biroh street, Mechanio.burg, Cumberland
county, Pannsylvania 170&5.
2. Respondent, Pennlylvania Department ot Tran.portation,
Bureau of Driver Lioeneing, hae a mailing address at Transportation
and Safety Building, Harr1eburq, Dauphin county, Penneylvania
17120.
3. Petitioner reoeived a citation tiled by the carlisle
Polioe Department on or about Ootober 15, 1993, alleqing a
violation on or about October 15, 1993, ot seotion 1543 ot the
Vlhicle Code.
4. The oitation was is.ued through Magilterial Dietrict 09-
2-01 in carli.le, Cumberland county, Pennsylvania.
o. Petitioner appeared betor. District Jl,lltice Correal,
Malli.ter1al Diatriot 09-2-01, on Novembtr 22, 1993, and the
Dietriot Juatioe accepted Petitioner'e guilty plea and accepted
Petitioner'a payment.
6. Petitioner received a notice ot l1cen.e lu.penaion with
a maU data of January 17, 1994, !l'om the D.partm.nt of
Tranaportation advidng her that her lioen.. waa to be aUepended
for a period ot one year with an effective date ot Harch 27, 1998.
Thia notice ie attached hereto and is inoorporated herein by
reference aa Exhibit "A".
