COMBS NOW John Robert Koonc~, petitioner in the abov~-
captioning, herein after referred to as "petitioner", who hereby
petitions this !lonorablo Court to grant petitioner's request to
legally change his name from "John Robert Koonce" to "Jeremy
Robert Domenico," and who st.ates the folloWing in support of
this petitionl
1. That petitioner is a resident of the Commonwealth
of Pennsylvania where he has been a resident for
a period of more than five (5) years,
2. That petitioner files this Petition in cumberland
County to eliminate any Jurisdictional Questions
and to invoke Jurisdiction for the Department of
Corrections which is also in Cumberland County;
3. That peti tioner r.as utili zed the name "Jeremy Robert
Domenico," here,in after "alias", for a period in
excess of fifteen years to which said alias was
used and recognized by this !lonorable Court for
the purpose of prosocution in 1962; (see footnoto "1)
4. That peti t ioner has ma rried under the alias name
of "Jeremy Robert Domonico" and has children under
the alias name of "Domenico";
5. That petl tloner appl ios in good faith and not for
the purpose of covering up or obscuring uny criminal

of my financial condition am unable to pay the fees and cost
that may be incurred in thiB action at hand.
