NOW COMES Flower, Morgenthal, Flower and Undsay, Plaintiff above, and states as follows:
1. Plaintiff is a professional corporation with offices at 11 East High Street, Carlisle,
Cumberland County, Pennsylvania 17013.
2. Defendant Is an adult individual residing at 6280 Carlisle Pike, Lot 139, Mechanicsburg,
Cumberland County, Pennsylvania 17055.
3. On or about April 22, 1991, Defendant requested Carol J. Undsay, Esquire, an attorney
with the Plaintiff, to represent him with regard to a driving under the influence charge docketed to
the Number 877 Criminal 1991.
4. Plaintiff undertook representation and provided services and advanced costs pursuant
to the representation through trial and sentencing on June 2, 1992.
5. Defendant agreed to pay for services at Plaintiff's hourly rate at the hourly rate
