PETITION FOR NAME CHANGE
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of Kelley Anne Mondi, respectfully represents:
1. That she is a resident of said Cumberland
County, having resided at 403 Brian Court,
Mechanicsburg, Pennsylvania, since April 1993.
2. That for a significant reason she is desirous
to change her name to Kelley Anne Stone.
3. That there are no judgments or decrees of like
character of record against the Petitioner.
WHEREFORE, Petitioner prays the Court to enter a decree
changing her name from Kelley Anne Mondi to Kelley Anne Stone.
I verify that the statements made in this Petition are true
and correct. I .understand that false statements herein are made
subject to the penalties of IS Pa, C.S. 5904 relating to unsworn
falsification to authorities.
