If you wish to deny any of the statements set forth in this
affidavit, you must file a counteraffidavit within twenty days
after this affidavit has been served on you or the statements
will be admitted.
PLAINTIFF'S AFFIDAVIT UNDER SECTION 3301(d)
OF THE DIVORCE CODB
1. The parties to this action separated on February 21,
1991 and have continued to live separ.ate and apart for a period
of at least two years.
2. The marriage is irretrievably broken.
3. I understand that I may lose rights concerning alimony,
division of property, lawyers' fees or expensos if I do not claim
them before a divorce is granted.
I verify that the statements made in this affidavit are true
and correct. I understand that false statements herein are made
subject to the penalties of 18 Pa.C.S. 54904 relating to unsworn
falsification to authorities.

Personally appearad before me, a Notary Public, in and for
laid Commonwealth and County, ROBERT B. LIEBERMAN, ESQUIRE, who,
being duly sworn according to law, deposes and sayst
1, That on March 17, 1994, a Complaint in Divorce,
properly endorsed with a Notice to Defend and Claim Rights and an
Affidavit Under Section 3301(d) of the Divorce Code were filed on
behalf of the Plaintiff and against Defendant in the above case.
:a, That on March 23, 1994, I forwarded, by certified mail,
return receipt requested, restricted delivery, a certified copy
of the Complaint in Divorce with a Notice to Defend and Claim
Rights, and the Affidavit Under Section 3301(d), to the
Defendant, JAMES P. STRAHOSKV, at 206 Locust Street, Camp Hill,
Cumberland County, Pennsylvania, as evidenced by the sender's
roceipt attached hereto.

Divorce with a Notice to Defend and Claim Rights and the
Affidavit Under Section 3301(d) , sent to the Defendant, JAMBS p,
