( 1) A Complaint in divorce under Section 3301 (c) of the
Divorce Code waD fi~ed on March 21, 1994.
(2) Defendant acknowledgee that service of the Complaint was

(3) The marriage of Plaintiff and Defendant is irretrievably
broken and ninety days have elapeed from the date of filing the
Complaint.
(4) I under~tand that I may lose righte concerning alimonr'
division of property, lawyer'a fees or expeneee if I do not cla 10
them before a divorce ie granted.
(5) I coneent to the entry of a final decree of divorce.
(6) I understand that if a claim for alimony, alimony
pendente lite, marital property or counsel feee or expensee has
not been filed with the court before the entry of a final decree
in divorGe, the right to claim any of them will be loet.
