Dear Kathryn:
I had the pleasure of meeting your patient PEGGY HAMPTON in the office October
27, 1992. Thank you for the referral.
As you know Peggy is a very pleasant 38-year-old woman with bilateral knee
problems dating back from an injury in May of this year in which she was involved
in a bicycle vs. motor vehicle accident. She did not fall from her bike but braced
herself against impact with both knees. She has had intermittent but persistent
knee problems since that time. Anti-inflammatory medications have not led to relief.
She is active in step aerobics but is limited to one to two days per week as recovery
time prevents more frequent participation. She states that she has noted more
clicking and cracking in her knees since the accident.
On examination she has a free range of motion of both knees. She has no effusion
today, She has slight tenderness in the patella tendon region. Her left knee shows
a Grade II-III Lachman's and small posterior drawer while the right knee shows a
Grade I Lachman's. McMurray's tests are normal, X-rays are essentially normal,
Peggy has ligament injuries of both knees, left worse than the right. She has at
least partially torn her left anterior crJciate ligament. For both knees I would
recommend a rigorous exercise program to strengthen the quadriceps, hamstrings and
gastroc and calf muscles in order to provid~.su~port for the knee that can no longer
be provided by her ligaments. Following this period of time I will recheck her and
determine if any type of bracing is needed.

Dear Kathr/n:
I saw our common patient PEGGY HAMPTON in follow-up today. December 8. 1992.
Ber knee pain is improved as has her strength. She is exercising vigorously
three times weekly.
On examination she has findings consistent with patellofemoral syndrome with
positive patellar grind and patellar tendon tenderness. I asked her to continue
her Lodine as she continues on the present program. I will recheck her in
approximately two months and decide if she needs a brace for her patellofemoral
symptoms.
Thanks again for allowing me to share in the care of this delightful woman.

Dear Katherina:
I saw Peggy Hampton in the office April 12, 1994. PeRgy's kneea have been
aching during and aftar certain activities. She has not been keeping up
with her knee exercises per say. Her mechanical symtpoms are not prominent
although she will notice that the knees will pop with certain twisting
maneuvers. The knees have not given out on her. She ~ able to cycle great
distances laAt summer without difficulty.
On examination today Peggy has a Grade II Lachmans with good
left and a Grade I Lachmans with good endpoint on the right.
is otherwise normal except for some quadriceps atrophy.

I had a long discussion 'with Peggy relating to her knees. I felt that any
of her symptoms could be coming from slightly increased laxity in her knees.
I reinforced the need for her to continue very rigorous ~xercise program.
This should keep her knees quite healthy. She will get more serious with
her exercises, and if this doesn't do the trick, she will let me know.
It's been a pleasure sharing in the care of this very nice lady.

Thank you for referring Ms. Peggy Hampton to me for evaluation of her problems
of persistent bilateral knee pain. This 38 year old woman who has been
very active physically over the years with aerobics and biking. developed
bilateral knee pain starting May 1992 after a bike accident. Although
she did not actuslly fall at that time, she did run into a curbe straining
her legs which were clipped into the bike pedals. Since that time, she
has had persistent pain localized predominstely to the anterior knee area,
particularly at the attachment of the pat~llar tendon to the tibial condyle
but also slightly more diffusely in the knees. During the work-up of this
problem, her x-rays have been normal and other blood tests negative but
the patient was discovered to have a slightly positive ANA in a titer of
1:80 spe~kled pattern with a negative double stranded DNA and SM & RNP
antibodies.
On careful questioning of the patient. she denies any inflammatory a~thritis
symptoms such as morning stiffness, joint swelling, fatigue, rash, Raynaud's
phenomena and pleuritic chest pain. In the past month and since starting
an active exercise program under the guidance of Dr. Frankeny, she has
noticed an improvement of bilateral knee pain.
My exam shows a well developed white female in no acute distress with normal
stance and gait, Her general physical exam was unremarkable. On musculoskeletal
exam, all joints showed full range of motion without swelling or tenderness.
With specific reference to the knee exam, the following was noted. There
was full range of motion of the knees with no joint swelling or effusion.
I was unable to document any crepitus on the exam. There was no extreme
smount of tenderness on patellar compression testing however, she did experience
mild discomfort at the attachment of the patellar tendon after repetitive
exercise. No definite ligamentus instability or locking of the knee was
noted. Her neurovascular exam was normal.
My impressions are that Hs. Hampton appears to have had a patellar tendonitis
of both knees et its attachment at the tibIal condyles which is improving
now. Despite the low titer positive ANA test, I could find no evidence
for an inflammatory arthritis or a connective tissue disease to explain

her symptoms.
Hy recommendations for her at this time are for her to continue with her
strengthening exercise as prescribed by Dr. Frankeny. I do feel it would
reasonable for her to taper off tha Lodine in the near future since it
may not be necessary for her to continue on this medication without definite
inflammation.
Hy impressions are that her prognosis for full recovery is very good and
she is likely to be able to resume full end active physical activity in
the very near future.

S: Peggy is in today stating she was hit by a car on 5/29/92,
while she was riding a bike. Her knee slammed into the curb.
She was seen at First Care, examined and placed on Naprosyn. No
x-rays were taken. She had not had any history of preVious trauma
to her knee. She states ever since, she still has quite a bit
of pain in her knees. The Naprosyn takes the edge off, but she
is still having a lot of pain. PMH-negative. Past surgical history-
negative. She does not smoke. Allergies-none known.
0: The patient is noted to not have an effusion over the
knee. She does have tenderness on deep palpation. No ecchymosis
or scrapes are seen. ROM is full. .<: /

LUMBAR SPINE, 6V:
The routine projections of the lumbar spine show that there are five
lum~ar vertebral bodieR. No significant a~omalies are identified. There
has been a minor attemrt at lumbarization of SI. The vertical height of
the vertebral bodies and intervertebral disc spaces appears to be well
maintained throughout. We were unable to identify evidence of osteophytic
chsnge or end plate sclerosis. The prominence of the anterior superior
margin of the t5 vertebral body, at the L4-5 interspace, appears to be
developmental. The sacroiliac joints Rre symmetrical without marginal
erosion or sclerosis. In the AP projection of the lumbar spine, there is
a tendency towards a mild right dorsa-lumbar scoliosis with the apex at
L2. This may be positional.
CONCLUSION: E~INATION OF THE LUMBAR SPINE SHC~S 0000 PRESERVATION
OF THE VERTICAL HEICHT OF TFE VERTEBRAL BODY AND
INTERVERTEBRAL DISC SPACES THROUCHOUT. WE WERE UNABLE TO
IDENTIFY ANY SIGNIFICANT ABNORMALITY.

knee joint compartments are not narrowed. I do not see scute bone
sbnormalities.
CONCLUSION: NORMAL LEFT KNEE.
RIGHT KNEE, 4V:
I do not see soft tissue swelling or knee joint effusion. I do not see
abnormal soft tissue calcifications, although there probably is a small
phlebolith posterior to the mid to lower femoral shaft. The knee joint
compartments are not narrowed. I do not see a fracture or other acute
abnormality.
CONCLUSION: NORMAL RIGHT KNEE.

abnormal soft tissue calcifications, although there probably is a small
phlebolith posterior to the mid to lower femoral shaft. The knee joint
compartments are not narrowed. I do not see a fracture or other acute
abnormality.

S: Peggy is in today stating she was hit by a car on 5/29/92,
while she was riding a bike, Her knee slammed into the curb.
She was seen at First Care, examined and placed on Naprosyn. No
x-rays were taken. She had not had any history of previous trauma
to her knee. She states ever since, she still has quite a bit
of pain in her knees. The Naprosyn takes the edge off, but she
is still having a lot of pain. PMH-negative. Past surgical history-
negative. She does not smoke. Allergies-none known.
0: The patient is noted to not have an effusion over the
knee. She does have tenderness on deep palpation. No ecchymosis
or scrapes are seen. ROM is fUIl.1
A: Contusion to knee . ,I,
P: Will check knee x-rays bilaterally. Trial of Orudis "
1 PO TID. Vicodin for pain. Recheck in 2 weeks, sooner if x-

Since my Oct. 27 appointment, I have been doing the exercises you
recommended. In just a short time frame, I am doing more and feeling less
discomfort in my knees! I found your diagnosis optimistic and not
limiting to my life style.
Attached are the test results of tests which Dr. Gallagher ordered
during my Oct. 23 visit to her office. I thought she was just doing a
cholesterol screening and was surprised by the tests she ran and upset by the
results of those tests. I wanted you to see the results of the tests so
you can determine if we are on the right treatment path. Dr. Gallagher
advised me over the telephone that I tested "positive for arthritis."
Also, after I left your office, I remembered that my paternal
Grandmother suffered from arthritis. (Her hand joints were swollen
and twisted.) I have never had any pain in mv knees prior to the
accident - even when I biked 200 mi les in 2 days 1100 mi-'dayl
Dr. Gallagher has recommended that I see a rheumatologist, Dr. Ro~n.
I wanted you to have this information to see if you want to make any
changes to your recommended treatment of my knee injuries.
Basically all of this is getting rather confusing, frightening and
depressing for me as I've planned on a very active future.
I'd like to talk to you in the next few days either by telephone or
office visit. I want to work with you to achieve best results. I would
like to discuss my treatment based on the new info. Also, are there any
specific questions which I should ask the rheumatologist? Does the new
info impact your field of expertise in treating me as a patient?
Please contact me as soon as possible. I believe that a healthy body
is linked to a healthy mind and right now my "mental state" isn't doing very
well.
I've included a statement regarding my health prior to the accident and
would like it retained in my file. My goal is to return to my previous
level of health and activity.

I WAS AN AEROBIC ANI ,UNTIL BIKE ACCIDENT ON MAY 29, 92. AN EXAMPLE OF MY
LEVEL OF PHYSICAL ABILITY IS MY BICYCLING. ON 21 AND 22 SEPTEMBER 1991 (JUST
OVER A tEAR AGO, I BIKED 185 MILES IN 2 DAY. ON THE FIRST DAY, I BIKED 100 MI.
AT THE 95 MI POINT, I WAS PHYSICALLY TIRED AND MENTALLY QUESTIONED MY SANITY
ABOUT PLANNING TO RIDE 85 MI THE NEXT DAY. WENT TO BED AT 8:30 THAT NIGHT AND
WOKE UP REFRESHED AND READY TO RI DE THE NEXT DAY. ABOUT 50 MI I ';TO THE RI DE,
WE STARTED TO RACE AND RODE A PACE LINE FOR THE BALANCE OF THE RIDE. WE
ARRIVED 45 MINUTES EARLY (BASED ON OUR SPEED OF THE PREVIOUS DAY). THIS IS
AN EXAMPLE OF MY RIDING ALL LAST SUMMER AND I EXPERIENCED NO PAIN IN MY KNEES
AND LEGS. THIS SPRING, I WAS BIKING BETWEEN 25-50 MI SEVERAL DAYS IN A ROW AT
AN AVERAGE SPEED OF 16-17 MPH WITH NO PROBLEM.
I WENT TO THE HEALTH CLUB ON A REGULAR BASIS. I DID AEROBICS 4, 5, OR 6 TIMES
PER W~EK AND NEVER EXPERIENCED ANY PAIN. I DID HIGH IMPACT AEROBICS, LOW
IMPACT, AND STEP CLASSES (USING AN 8 INCH BENCH). I NEVER FELT PAIN IN MY
KNEES AND LEGS AND WAS SO WELL CONDITIONED THAT I RARELY HAD ACHES IN MUSCLES IN
MY BODY. I DID THIS LEVEL OF ACTIVITY AT LEAST 2 YEARS PRIOR TO THE ACCIDENT
ON HAY 29, 1992.
THE ACC IDENT:
I WAS BIKING THROUGH MECHANICSBURG WHEN A LADY CUT ME OFF. HER CAR CONNECTED
WITH MY HANDLE BARS AND PULLED ME IN TOWARD THE CURB. I WAS UNABLE TO STOP AND
SLAMMED INTO THE CURB. THE IMPACT BLEW OUT MY FRONT TIRE. MY CYCLOCOMPUTER
FLEW ABOUT 6 FT AND MY FRONT WATER BOTTLE FLEW OUT. I NEVER LEFT THE ,IKE.
MY FEET WERE IN TOE CLIPS (THE KIND THAT FORK ACROSS TOES). WHILE MY FEET CAME
TO A INSTANT HALT, MY UPPER BODY PROBABLY CONTINUED IN MOTION WITH A LOT OF
PRESSURE INTO MY HAND, WRISTS, AND ARMS. INITIALLY, MY LEGS DIDN'T HURT MUCH,
BUT AN HOUR LATER WHEN I GOT HOME, MY LEGS STARTED TO FEEL KIND OF NUMB SO I
WENT TO SEIDLE FIRST CARE. THE DR. GAVE ME A PERSCRIPTION FOR NAPROSON AND
SAID IF I WAS STILL IN PAIN IN A WEEK TO SEE MY FAMILY DR.
ONE WEEK LATER, I WAS IN A LOT OF PAIN IN THE FRONT OF MY KNEES. THE PAIN
SEEMED TO BE IN BOTH KNEES FROM THIGH TO ANKLE LEVEL. SOMETIMES I FEEL PAIN IN
MY HIP JOINTS. ALL OF THIS PAIN HAS STARTED AFTER THE BIKE ACCIDENT.

caused or contributed to by the negligence, carelessness and recklessness of the
Plaintiff and that she was attempting to pass the Defendant on the right hand side
between the tl'8vellane and the parking lane of West Simpson Street.
11. The Plaintiff, Peggy Hampton, assumed the risk of her activities and
of injury as a result thereof.

Momingslde Drive, Camp Hill, Cumberland County, Pennsylvania,
2, The defendant, Bunnie L. Pinti, Is an individual residing at 623
Gutshall Road, Boiling Springs, Cumberland County, Pennsylvania,
3. On May 29, 1992, at or about 6:20 P,M., the plaintiff was operating
her bicycle in a western direction in the first block of West Simpson Street in
Mechanicsburg, Cumberland County, Pennsylvania.
4, At the same time and place, the defendant, Bunnie L. Plnti, was
operating a 1988 Nlssan sedan, behind the plaintiff, attempted to pull into a
parking place, striking the plaintiff, and forcing the plaintiff into the curb,
5. The collision was caused by the negligence of the defendant In the
following respects:
safety;
(a) in changing lanes without seeing that it could be done in
(b) in misjudging the location of the plaintiff, and
(c) in failing to have her car under control.
6. As a result of the negligence of the defendant, the plaintiff
sustained ligament injuries of both knees, and a torn left anterior cruciate
ligament.
7, As a result of the aforesaid injuries, the plaintiff has undergone
great p~ and suffering, and will do so in the future.
8, As a result of the aforesaid injuries, the plaintiff has incurred a
permanent, partial disability of her knees,
9, As a result of the aforesaid injuries, the plaintiff has undergone
extensive medicaltreabnent and physical therapy,

WHEREFORE, the plaintiff claims of the defendant a sum in excess of the
arbitration limits of the local rules of Court.
