1. The name of the municipality by whioh this claim
i. filed is the HAHPDEN TOWNSHIP SEWER AUTHORITV, a
municipal authority of Hampden Township, Cumberland County,
Pennsylvania, with offices at 230 SportinB Hill Road,
MeohanioeburB, Cumberland County, Pennsylvania 17055.

Deed, dated September 19, 19S5, and recorded in
the Cumberland County Recorder of Deeds Office in
Deed Book 31, Volume H, PaBe 478, alkla 5259

4. The authoriti.. under and by virtue of whioh the

of Kay 2, 1946, P.L. 3S2, as amended and .upplementedi
Re.olution of Ha.pden Town.hip Sewer Authority No. 93-02,
adopt.d July 14, 1993 as amended. The olaims for oollection
of fe.. are based upon the Act of the A..embly approved Kay
16, 1923, P.L. 207, as amended and supplemented.
5. All acts, oonditions, events and thin.s required
to be done under the Ordinance of the Township of Hampden,
R..olutions of the Hampden Township Sewer Authority, and
Act. of the Aseembly of the Commonwealth of Pennsylvania,
have b.en and were performed in due and leBal form, to
entitle the Hampden Town.hip Sewer Authority to file a li.n
for the pay.ent of the oontribution fee for whioh this claim
i. filed.
6. The within claim arises for the balanoe due for
Contribution Fee oblisated under the Sewa.e Connection
Permits No. 21S1 and 21S2, dated Hay 25, 1993. See Ellhibit
"A" attached hereto and incorporated herein by reference.
The payment of One Thousand Two Hundred Fourteen Dollars
(t1,214.00) was paid on Harch 1, 1994, leavinB a balance of
Five Thousand Seven Hundred Sillty Dollars (t5,760.00) due
and owinB.
