VINOD VASISTH, Defendant CIVIL ACTION - LAW COIlPLun AND NOW comes the plaintiff, Chase Manhattan Bank, N.A., by and through its attorneys, Turner and o'Connell, and makes the following complaint: 1. Plaintiff is Chase Manhattan Bank, N.A., a lending institution having its principal place of business at 900 stewart Avenue, Garden city, New York 11530. 2. Defendant is Vinod Vasisth, an adult individual whose last known address is 441 sioux Drive, Mechanicsburg, Pennsylvania 17055. 3. On or about January 20, 1978, the defendant made application to the plaintiff for the issuance of a credit card. A true and correct copy of that crsdit card application is attached hereto as Exhibit "A". 4. Pursuant to defendant's application, plaintiff issued defendant a Chase Manhattan VISA card, account number 4225 975 387 035. 5.
