TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of MEGAN EDITH LOUISE MULLEN, by her attorney, R.
MARK THOMAS, Esquire, respectfully represents as follows:
1. Petitioner was born May 24, 1978, and her birth
certificate reflects the name of MEGAN EDITH LOUISE MULLEN.
2. Petitioner's biological father's name is navid Konop and
Petitioner's biological mother's name at the time of her birth was
JoAnn Veronica Mullen.
3. Petitioner has had no contact with her biological father
since her date of birth, and has continuously resided with her
mother.
4. Petitioner's mother was married on October 13, 1979 and
now goes by the name of JoAnn Veronica Darrow as a result of her
marriage.
5. Although Petitioner's last name as stated on her birth
certificate is MULLEN, Petitioner has always used the last name of
DARROW since her mother's marriage in 1979.
6. Petitioner now desires to legally change her name to
MEGAN EDITH LOUISE DARROW for the following reasons:
a. Petitioner has used the last name of Darrow since
