filing in writing with the court your defenses or objections to the

money claimed in the complaint or for any other claim or relief

COMES NOW, the Plaintiff, by and through his attorneys, ALLEN
SHAFFER & ASSOCIATES, and states the following:
1. Plaintiff, WOODROW R. HOFFMAN, is an adult individual
currently residing at 304 West Maple Avenue, Shiremanstown,
Pennsylvania, 17011.
2. Defendant, ALLSTATE INSURANCE COMPANY, is a Pennsylvania
Corporation with principal place ~f business located at the Gateway
Corporate Center, 6345 Flank Drive, Suite 1000, HarriSburg,
Pennsylvania, 17112.
3. Defendant, HOLY SPIRIT HOSPITAL, is a Pennsylvania
Corporation with principal place of business located at 503 North
21st Street, camp Hill, Pennsylvania 17011.
COUNT J:
PLAJ:NTIFF v. HOLY SPIRIT HOSPJ:TAL
4. On or about October, 1993, Gladys Hoffman, wife of the
Plaintiff, was admitted to Defendant, HOLY SPIRIT HOSPITAL, to
undergo treatment.
5. On or about October 18, 1993, Plaintiff came to the
Defendant, HOSPITAL, to visit his bedridden wife. He observed that

the decedent's hand was bloody and that his wife's diamond and
wedding rings were on her hand.
6. Plaintiff asked an employee nurse of Defendant, HOSPITAL,
to clean the hand.
7. Later the same day, after the hand had been cleaned,
Plaintiff observed that the aforementioned rings were missing.
8. Plaintiff and a friend, fearful that the rings had been
stolen, searched the decedent's hospital room, looking through the
room, the floor, and the waste baskets.
9. The rings have never been found.
10. The Plaintiff's wife was bedridden and could not have
misplaced the rings.
11. In February, 1994, the Plaintiff's wife died.
12. Defendant, HOLY SPIRIT HOSPITAL, has thus far denied
responsibility for the lost rings.
WHEREFORE, the Plaintiff requests this Honorable Court to
award him $2,500 plus costs, attorney fees and interest.
COUNT II
PLAINTIFF V. ALLSTATE INSURANCE COMPANY
12. Paragraphs 1-12 are incorporated herein by reference.
13. Defendant, ALLSTATE INSURANCE COMPANY, is the Plaintiff's
homeowner's insurance carrier.
14. After conducting a thorough search, and failing to find
the rings, the Plaintiff submitted a claim for his loss to
Defendant, ALLSTATE INSURANCE COMPANY.
15. Until Defendant, HOLY SPIRIT HOSPITAL, informed it that
it could not state with certainty whether the rings had been stolen
-
or merely misplaced, Defendant, ALLSTATE, was prepared to pay the
Plaintiff $2,000 for the rings.
16. Defendant, ALLSTATE, continues to deny the Plaintiff's
claim.
17. The missing rings are valued at approximatelY $2,500.
18. The Plaintiff has not yet been compensated for his loss.
WHEREFORE, the Plaintiff requests this Honorable Court award
him $2,500 plus costs, attorneys fees and interest.

R. Thomas Kline, Sheriff who being duly sworn according to law,
says he made diligent search and inquiry for one of the within named
defendants to wit: Allstate Insurance Company, but was unable to locate
them in his bailiwick. He therefore deputized the sheriff of Dauphin
County to serve the within Civil Action Law Complaint according to
law.
DAUPHIN COUNTY RETURN:And Now June 14, 1994 at 3:15 P.M. served the
within Complaint upon Allstate Insurance company, by personally handing
to Marlene Wright, Receptionist, and person in charge at time of service,
a true and attested copy of the orignial Complaint, and making known unto
her the contents thereof at 6345 Flank Drive. Suite 1000, Harrisburg, Pa.

Dauphin County return is hereto attached.
Michael E. Barrick, Deputy Sheriff who being duly sworn according to
law, easy on June 14, 1994 at 1:15 o'clock P.M.E.D.S.T., he served a true
copy of Civil Action Law Complaint. in the above entitled action upon
the within named defendant to wit: Holy Spirit Hospital, by making known
unto Sister Rose Willam, person in charge at time of service at Holy Spirit
Hospital, 503 North 21st Street, Camp Hill, Cumberland County, Pennsylvania,
its contents and at the same time handing to her personally the said true
and attested copy of the same.
