Notice is hereby given that the Defendant in the above-captioned matter, having been
granted a Final Decree in divorce from the bonds of matrimony on the 15th day of March, 1995,
hereby elects to retake and hereafter use her previous name of Fern Irene Bosserman, and gives
this written notice avowing her intention in accordance with the provisions of the Act of May 25,
1939, P.L. 192 as amended.
