5. The surname "Hiller-Latham" was the surname of the
Petitioner at the time of birth of her daughter.
6. The putative father of Jessica Elaine Hiller-Latham is
David Hark Wiener whose last known address is 9023 Glen Creek
ct't Dallast TX 75243.
7. The Petitioner has never attempted to legally establish
the paternity of her daughtert nor has she ever attempted to
receive child support for her daughter from the putative father.
B. The putative father has never attempted to establieh
rights of visitation with Jessica Elaine Hiller-Latham.
9. Petitioner's daughter has never had contact with the
putative father since her birth, and no parent-child relationship
has ever been established.
10. It would be in the best interest of Jessica Elaine
Miller-Latham to have her name changed to Jessica Elaine Jaccard
because I
a. Jessica Elaine Miller-Latham has resided in the
Jaccard household for eleven yearsl
b. Jessica's mother and half-sister have the surname
Jaccardl
c. Jessica has never had contact with her putative
father and views the petitioner's husband, John Ksnneth Jaccardt
as her father,
d. A change of name to Jaccard fosters the concept of
a family unit.
