COMPLAINT
The Plaintiff, United Guaranty Residential Insurance Company of North Carolina,
Assignee of City Savings FSB F/KJA City Consumer Services, Inc., by Its attorneys.
KNUPP. KODAK, P.C., brings this action of Assumpsit against t!"le Defondant to recover
the sum of FIVE THOUSAND NINE HUNDRED NINE DOLLARS AND NINETY-FOUR
CENTS ($5,909.94), along with Interest thereon at the rate of fourteen and one half
percent (14,5%) per annum from February 28, 1994, upon a cause of action of which the
following Is a statement:
1. The Plaintiff, United Guaranty Residential Insurance Company of North
Carolina, Assignee of City Savings FSB F/KJA City Consumer Services, Inc., is a
corporation organized and existing under the laws of the State of North Carolina, having
Its principal office and place of business at 230 North Elm Street 27401, P.O. Box 20327,
Greensboro, North C6I'0llna 27420-0327.

2. The Defendant, Robert C. Bales, Is an adult Individual residing at 22 Charles
Street, Carlisle, Cumberland County, Pennsylvania 17013.
3. On June 6, 1983, the Defendant did borrow $14,000.00 from Plaintiff's
assignor for a home Improvement and bill consolidation loan as per Exhibit "A" attached
hereto and made a part hereof.
4. Defendant did make the required payments until November 10, 1990,
leaving a principal balance due and owing of THREE THOUSAND NINE HUNDRED
THREE DOLLARS AND FIFTEEN CENTS ($3,903.15) plus Interest of ONE THOUSAND
TWENTY-ONE DOLLARS AND EIGHTY CENTS ($1,021.80) up through February 28,
1994. Interest continues to accrue at ONE DOLLAR AND FIFTY-SIX CENTS ($1.56) per
day from February 28, 1994,
5. Per the terms of the Note found at Exhibit "A", Defendant Is also responsible
for reasonable attorneys' fees which are In the amount of NINE HUNDRED EIGHTY.
