AND NOW, this ~ day of ~, 1994, upon
agreement of the parties, it is hereby ordered and decreed that
the within Custody stipulation and AGreement is made an Order of
Court
1. The Fathar and Mother shall have shared legal custody of
the child, meaning that they shall share the legal right to make
major decisions affecting the best interests of the child and all
of the major matters related to the child, including, but not
limited to, medical, religious, and educational decisions.
2. Grandmother shall have primary physical custody of the
child.
3. Father and Mother shall have partial custody and
visitation with the child at times agreeable between the parties.
4. No party shall do anything which may estrange the child
from the other paries or injure the opinion of the child as to
the other parties or which may hamper the free and natural
development of the child's love and affection for the other

CUSTODY STIPULATION AND AGREEMENT
MADE this I> 7" day of T;. C '1 , 1994 , by and between
Wanda K. Bagshaw, hereinafter "Grandmother", Jeremy P. Borawski,
hereinafter "Father" and Tammy L. Bagshaw, hereinafter "Mother".
WHEREAS the Father and Mother had born to them a child,
namely, Paige N. Borawski, born April 14, 1994, hereinafter
"Child", and
WHEREAS the parties wish to enter into an agreement relative
to custody, partial custody, and visitation of the child, and
WHEREAS the child has resided in Cumberland County/
Pennsylvania continuously since birth, thus giving this Court
jurisdiction,
NOW THEREFORE, in consideration of the mutual covenants,
promises and agreements as hereinafter set forth, the parties
agree as follows:
1. The Father and Mother shall have shared legal cU8tody of
the child, meaning that they shall share the legal right to make
major decisions affecting the best interests of the child and all
of the major matter8 related to the child, including, but not
limited to, medical, religious, and educational decision..
2. Grandmother shall have primary physical custody of the
child.
3. Father and Mother shall have partial custody and
visitation with the child at times agreeable between the parties.
4. No party shall do anything which may estrange the child
from the other paries or injure the opinion of the child as to
the other parties or which may hamper the free and natural
development of the child's love and affection for the other
parties.
5. The failure of any party to insist on strict performance
of the provisions of this stipulation and agreement shall not be
construed as a waiver of any subsequent default of the same or
similar provision.
6. The parties agree and desire that this stipulation and
agreement be made an Order of Court by the Court of Common Pleas
of Cumberland County, Pennsylvania.
7. This Stipulation and Agreement shall become an Order of
Court with the Court of Common Pleas of Cumberland County,
Pennsylvania, entering said Order and retaining jurisdiction
should any party desire or require modification of said order.
