AFFIDAVIT OF ~~
1. A Complaint in Divorce under Section 3301(c) of the
Divorce Code was filed on July 26, 1994.
2. The marriage of Plaintiff and Def~ndant is
irretrievably broken and ninety (90) days have elapsed from the
date of filing the Complaint.
3. I consent to the entry of a final Decree of Divor.ce,
4. I understand that if a claim for Alimony, Alimony
Pendente Lite, Marital Property, or Counsel Fees or Expenses has
not been filed with the Court before the final Decree in Divorce,
the right to claim any of them will be lost,
5, I understand that I have the right to request that my
spouse and I participate in marriage counseling. Being so advised
I do not request marriage counseling.
I verify that the statements made in this Affidavit are
true and correct, I understand that false statements herein are
subject to the penalties of 16 Pa. C,S. Section 4904 relating to
unsworn falsification to authorities,
