Your Petitioners are Christopher R. Paparone and Carrie L. Paparone, adult
individuals currently residing at 1005 Shannon Lane, Carlisle, Cumberland County,
Pennsylvania.

Petitioner, Carrie L. Paparone, is the natural mother of Phillip Scott Morris, a minor
bom July 24, 1990.
Petitioner Carrie L. Paparone is the natural mother of a minor child, Kimberly Ann
Morris, born March 14, 1985, the sibling of Phillip Scott Morris.
The natural father of the child is Richard D. Morris, an adult individual currently
residing at 5014 Arriola Lane, Lumberton, TX 71657.
The natural mother and natural father have been separated since 1990.
The Petitioners were married on December 21, 1992.
AND NOW comes Petitioners, Christopher R. Paparone and Carrie L. Paparone, by and
through their counsel of record, Marylou Matas, Esquire, and petitions the Court as follows:
The child has resided with Petitioners at that address for the past two years, and with
Petitioners at 574 Craig Road, 14 Rear, Carlisle, Cumberland County, Pennsylvania,
for three years prior.
There has been no contact between the child and the natural father for more than two
years.

The Petitioners are the natural parents of a third minor child, Carl Ross Paparone,
who also lives with Petitioners and the minor child at issue.
10. The Petitioners and the child at issue have lived together as a family since 1992.
11. Petitioners believe and, therefore, aver that it is in the child's best interest to have the
same last name as Petitioners, that being Paparone.

The child desires that his last name be changed from Morris to Paparone.
The child at issue is enrolled in Carlisle Area School District and the Defense
Eligibility Enrollment System for military dependent benefits under the last name of
Paparone.

The Petitioners wish to have the same last name as the child and to be legally able to
stop using the last name of Morris for the child in order to promote their family unit
and maintain consistencies for legal documents, such as school transcripts, medical
records and college applications.
15. The child at issue is not the owner of real estate or any other real property.
16. The child at issue does not have any creditors.

The minor child, Phillip Scott Morris', State Police fingerprint card is not attached to
this Petition because he is 12 years of age or younger and meets the exceptions set
forth in 23 Pa.C.S.A. {}5105.
WHEREFORE, Petitioners request your Honorable Court to schedule a time and date for
a hearing with their request for a name change and further that the Court direct that notice of the
hearing be published one time for two consecutive weeks in the Sentinel, the newspaper of
general circulation in Cumberland County, and the newspaper of general circulation in
Lumbreton County, Texas, and by certified mail, restricted delivery to the natural father of the
child at issue, Richard D. Morris.

consideration of the Petition heretofore filed and the hearing had thereon, and the Court
having caused an investigation to verify the statements in the Petition and other facts to
give the Court full knowledge as to the desirability of the proposed name change; and
The Court being satisfied, finds that the statements made in the Petition are tree,
decrees the proposed name change so that PHILLIP SCOTT MORRIS shall be known as
PHILLIP SCOTT PAPARONE from this day forward.
