1. Claimant, Thomas D. White, individually and t/dfb/a T,D,W, Construction
("Claimant"), resides and maintains business offices at 806 Chambers Street, Breasler,
Pennsylvania. 17113 and files this Mechllnics' Lien Claim as contractor,
2. The owner of the property subject to the lien is Gettys Builders, Inc,
("Owner"), a Pennsylvania corporation with offices at 1901 State Street, Camp Hill,
Pennsylvllnia.
3. The date on which Claimant completed the work for which this claim Is
made was on or about July 5, 1994,
4. The Claimant contracted with Owner to supply labor and work for tbe
original erection and construction on the building of the property described in paragrapb
6 herein.
5, The nature and character of the labor furnished pursuant to the contract
was to provide trim, Installation and ]abor for a new home as set forth on Claimant's
Invoices numbered 05178 and 05195, true and correct copies which are attached hereto
as Exhibit A and incorporated herein by reference, The sum claimed to be due for the
labor 15 $2,928,00, plus Interest thereon on the unpaid balance from the date of the
completion of the structure,
