'lllis Agrecment is entered on this /,0 I~. day of Beptear,
1994, by the Plaintiff, Drian S. Hutkowski, and the Defendant, Lurlei K.
Rutkowl;lki. 'Ille Plaintiff / hereinaftllr referred to as "~'ather" and the
Defendant, hereinafter referred to as "Mother", agree to the entry of an
Order providing the following custody arrangements for their Child, Alex
arian Rutkowski, born NI~ven1ber 1, 1992r
1. f'ather and Mother shall share legal custody which is the legal
right to make major decisions affecting the best inter<'lst of the
Child including medical, religious and educational decisions.
2. Fathar shall have prin\~ry physical custody of the Child.
3. Mother shall have partial physical custody of the Child according
to the following schedule:
(a) Mother shall have partial physical custody every other
weekend from f'dday at 5:00 p.m. until Sunday at 8100 p.m.
The weekend custody pedods shall correspond wit;h those
weekends on which Mother also has custody of her daughter,
Alicia.
(b) Mother shall have partial physical custody every Tuesdl\Y
from 5rOO p.m. to 9rOO p.m.
(c) Physical custody over holidays shall occur on an alternating
basis as arranged by mutual consent of Mother and Father und
shall include New Year's Day, Easter, Memorial Day, Fourth
of July, Labor Pay, ThanksgiVing, Christmas Eve and
Christmas.
(d) Mother shall have part ial physical custody of the Child on
every Mother's Day and the Child shall remain in the
physical custody of Father on every ~'ather I 13 Pay.
(e) Mother and Father shall share physical custody of the Child
on the Child's birthday. I3pedfic arrangements shall be
made by the mutual agreement of the parties pdor to the
child's birthday each year.
(f) Mother shall have partial physical custody of
the Child ever.y sUl1Y11er for one entire week
