attorney, Edward J. Weintraub, Eaquire, who states the following in
support of the within Complaintl
1. Plaintiff is David J, Dorotzak, an adult individual who
currently resides at 7228 Hammett Road, 'rampa, Florida 33647.
2. Defendant is Angela M, Dorotzak, an adult individual who
cur.rently resides at 3520 September Drive 4, Camp Hill,
Pennsylvania 17110, since approximately November of 1993.
3. Both the Plaintiff and the Defendant have been bona fide
residents of the Commonwealth of Pennsylv~nia for a period of at
least six (6) months immediatel.y preceding the filing of this
Complaint,
4. Neither Plaintiff nor Defendant is in the military or
naval service of the United States or its allies.
5. Plsintiff and Defendant were marrJ.ed on July 2'7, 1991
, at Middletown, Pennsylvania.
6, Thare have been no prior actions for divorce or annulment
of the marriage between the parties instituted by either of the
parties in this or any other jurisdiction.
7. Plaintiff has been advised that oounseling is availablQ
and that plaintiff may have the right to request that the court
require the parties to participate in counseling.

REQUEST FOR A NO-FAUL'I' DIVORCE UNDli:R
~ECTION 3J.Q1.Lq) OF TilE DIVORCE CODE
8, The prior paragraphs of this Complaint are incorporated
herein by reference thereto.
9. The marriage is irretrievably broken,
10, The parties have lived separato and apart since on or
about August 1992,
11. After ninety (90) days hnve elapsed from the date of the
filing of this Complaint, plaintiff intends to file an affidavit
consenting to a divorce, plaintiff believes that Defendant may
also file such an affidavit.
WHEREFORE, if both Parties file affidavits consenting to 8
divorce after ninety (90) days have elapsed from the date of the
filing of this Complaint, Plaintiff respectfully requests the Court
to enter a Decree of Divorce pursuant to Section 3301(c) of the
Divorce Code.

EQUITABLE DISTRIBUTION

1. A Complaint In Divorce under Section 3301(c) of the
Divoroe Code was filed on August 12, 1994,
2. The marriage of Plaintiff and Defendant is irretrievably
broken and ninety (90) days have elapsed from the date of the
filing of the Complaint.
3. I consent to the entry of a rin/,ll Decree of Divorce.
4. I understand that I may lose rights concerning alimony,
division of property, lawyer's fees or expenses if I do not CLaim
them before 6 divorce is granted,
5, I have been advised of the availability of marriage
counseling, and being so advised, I do not request that the court
require my spouse and myself to participate in merriage counseling.
I verify that the statements made in this Affidavit are
true and conect. I understal,d that false statements herein are
made subject to the penalties of 16 Pa,C,S, Section 4909 relating
