1. Plaintiff is MARY R. CARREIRA, who currently
resides at 409 Delancey Court, Mechanicsburg, Cumberland County,
Pennsylvania since August, 1993.
2. Defendant is WILLIAM E. CARRREIRA, who currently
resides 409 Delancey Court, Mechanicsburg, Cumberland County,
Pennsylvania, since August 1993.
3. Plaintiff has been a bona fide resident of the
Commonwealth of Pennsylvania for at least six (6) months
immediately previous to the filing of this Complaint.
4. The Plaintiff and Defendant were married on the
November 9, 1968, at Watertown, New York.
5. There have been no pending actions of divorce or
for annulment between the parties.
6. The marriage is irretrievably broken.
7. Plaintiff has been advised that counseling is
available and that Plaintiff may have the right to request that
this Honorable Court require the parties to participate in
counseling.

1. The parties hereto are husband and wife, a divorce action having been filed
to the above referenced term and number, and that it is anticipated by the parties that a Divorce
Decree will be entered in the near future In this action;
2. Plaintiff, MARY R. CARREIRA, has the following social security number,
087-42-6481. Plaintiff's current and last known mailing address is the Clinton Court Apartments,

3. Defendant, WILLIAM E. CARREIRA, has the following social security

4. Issues relating to equitable division of marital property pursuant to the
Pennsylvania Divorce Code Section 23 Pa.C.S. ~101 et seq., have been negotiated by the
parties, and have been formalized In a Divorce and Settlement Agreement, between them dated

August 20, 1994. WilLIAM E. CARREIRA, Defendant, hereinafter referred to as "DEFENDANT"
or "PARTICIPANT", Is employed by G. S. Electric, a unit of General Signal Corporation, and Is
a participant In the savings and stock ownership plan;
IT IS HEREBY ORDERED, ADJUDGED, AND DECREED AS FOllOWS:
1. That the portion of the aforementioned savings and stock ownership plan
with G. S. Electric, which was accumulated prior to the date of this Order, Is martlal property
subJect to distribution by this Court.
2. In accordance with the marital settlement agreement between the parties,
Plaintiff Is entitled to be paid at the earliest possible date, 100% of Defendant's Interest In the
value of the aforesaid plan as of August 20, 1994, provided that said 100% share may be
reduced to the extent that there may be any administrative costs or penalty for early withdrawal
from the plan.
3. The plan to which this Order applies Is the savings and stock ownership plan
of General Electric, a Unit of General Signal Corporation, or any successor plan,
4. Plaintiff, the ultimate payee, shall have the same rights with regard to her
portion of the plan as are available to Defendant, the participant, with regard to the plan. These
rights Include, but are not limited to the right to designate a beneficiary of retirement benefits In
any right to elect from then existing retirement dates and payment options. In no event shall the
ultimate payee have greater rights than those which are available to the participant. The ultimate
payee Is not entitled to any benefit not otherwise provided under the plan.

5. In the event that actuarial computation Is necessary to determine actuarial
equivalents and/or the difference between benefits actually accrued, non-subsidized benefits,
or employer subsidized benefits, the plan administrator shall obtain the services of any actuary
who was enrolled under subtitle .C' of Title 3, of the Employment Retirement Security Act of
1974.
6. Any reasonable costs Incurred by the plan administrator to effectuate the
terms and provisions of this Qualified Domestic Relations Order shall be assessed against the
parties such that each pays 50% of the balance of the costs.
7. The parties shall promptly notify the plan administrator of any change oflhelr
addresses from those set forth In this Order, and the parties shall promptly submit this Order to
the plan administrator for determination of its status as-a-
