You have been sued in court. If you wish to defend against!

COMPLAINT IN DIVORCE
AND NOW, comes the above-named Plaintiff, Brian Ammerman, by
and through his attorneys, The Law Offices of Patrick F. Lauer,
Jr., James K. Jones, Esquire, and makes the following Complaint in
Divorce:
1. The Plaintiff, Brian Ammerman, is an adult individual who!
currently resides at 10 Hollinger Street, Mount Holly Springs, '
Cumberland County, Pennsylvania, 17065.
2. The Defendant, Cynthia Ammerman, is an adult individual who
currently resides at 113 Bermond Avenue, Endicott, New York, 13760.
3. The Plaintiff has been a resident of the Commonwealth of
Pennsylvania for at least six months immediately prior to the
filing of this Complaint.
4. The Plaintiff and the Defendant were married on June 21,
1986, in Marietta, Georgia.
5. There have been no prior actions of divorce or for
annulment between the parties.
6. The marriage is irretrievably broken.
7. Plaintiff has been advised that counseling is available and
that Plaintiff may have the right to request that the court require
the parties to participate in counseling.

AFFIDAVIT OF CONSENT
1. A Complaint in Divorce under Section 3301(c) of the
Divorce Code was filed on September 20, 1994.
2. The marriage of Plaintiff and Defendant is irretrievably
broken and ninety (90) days have elapsed from the filing of the
Complaint.
3. I consent to the entry of a final Decree in Divorce.
4. I have been advised of the availability of marriage
counseling, that I may request the Court to require that my
spouse and I participate in counseling, and that the Court main-
tains a list of marriage counselors in the Prothonotary's Office,
which list is available to me upon request. Being so advised, I
decline to request that the Court require that my spouse and I
participate in counseling.
5. I understand that I may lose ri9hts concerning alimony,
division of property, lawyer's fees or expenses if I do not claim
them before a divorce is granted.
I VERIFY THAT THE STATEMENTS MADE IN THE FOREGOING AFFIDAVIT
ARE TRUE AND CORREC'T. I UNDERSTAND THAT FALSE STATEMENTS HEREIN
