3. The Defendant, Carla S. Polly, Is an adult Individual residing at 30 Miller
Street, Apt. C-4, Lemayne, Cumberland County, Pennsylvania 17043.

4. On September 10, 1992, the Defendants made, executed and delivered to
the Plaintiff a Note In the amount of EIGHT THOUSAND FOUR HUNDRED NINETY-FIVE
DOLLARS AND SEVENTY-EIGHT CENTS ($8,495.78), a copy of which Is attached
hereto as Exhibit "A" and made a part hereof.
5. Having delivered the said Note to the Plaintiff, the Defendants became liable
for the payment of said Note In accordance with the terms thereof.

6. The Defendants have failed to make regular payments on said Note and has
since become in default 01 the payment of said Note and the entire balance according to
the terms and conditions thereof Is due.

7. The outstanding balance as of September 20, 1994, on said Note II SIX
THOUSAND SEVEN HUNDRED TWENTY-EIGHT DOLALRS AND SIXTY-FIVE CENTS
(S8,728.ee), as shown on Plaintiff's Statement of Account, attached hereto as Exhibit '8'
and made a part hereof,
8. Due to Defendants' default In payments, Interest has been added In the
amount 01 ONE HUNDRED SEVENTEEN DOLLARS AND THIRTY-EIGHT CENTS
($117.38) to which Defendants are additionally liable.
9. In ac(:ordance with the terms and conditions of said Note, Defendants
promised to pay to the Plaintiff reasonable attorneys' fees and. accordingly, now Is
additionally Indebted to the Plaintiff in the amount of ONE THOUSAND THREE
HUNDRED EIGHTY-EIGHT DOLLARS AND NINETY-NINE CENTS ($1,388.99).

10. Plaintiff has frequently demanded payment from Defendants 01 sald amount
due and owing as aforesaid, but Defendants have refused and neglected and stili refU18
and neglect to pay said amount of any part thereof.

WHEREFORE, Plaintiff demands Judgment against the Defendants In the sum of

(13.9%) from September 20. 1994.
