5, The Defendant subsequently received a cash advance from
Plaintiff and incurred additional obligations in the sum of
$6,218.79, no part of which has been paid since Septembe~ 26, 1994
although duly demanded. A true and correct copy of a statement of
account in connection with said cr.edit card is attached hereto,
made a part hereof and marked Exhibit "C".
6. Notwithstanding the frequent demands by Plaintiff for
payment of the amount due, there has been a failure and refusal by
the Defendant to pay the same or any part thereof.
7. In accordance with the terms of Exhibit "B" Defendant
agreed to pay the plaintiff a finance charge on all sums due at an
annual percentage rate of 16.8% and Plaintiff is entitled to
additional finance charges from September 26, 1994.
8. In accordance with the terms of Exhibit "B", Defendant
agreed to pay to Plaintiff a reasonable attorney' B fee if the
account was referred to an attorney for collection, and Plaintiff
will incur an attorney's fee in the amount of $1,243.00.
WHEREFORE, Plaintiff demands judgment against the Defendant
in the sum of $7,461.79 plus interest from September 26, 1994 and
cost of this action.

Bank and Is authorized to take this affidavit on Its behalf and that the fKts set forth in the
forqolnl complaint are true and comet to the best of her Information, knowledle and belief.
She further states the annexed account lIiainst RHODY K, ROSS is cometly copied from the
books of orilinal entry of First Deposit National Bank that the charles were made in said books
at or about the time of their respective dates; that the charles arc comet and the account is just
and true as stated; that there is now due and owina thereon the sum of $6218.79 plus interest
and attorney fees; that no part of said sum has been paid or in any manner :settled; ar.d that there
are no deductions or offsets of any kind, except as are therein specified and credited, She
further understands that false statements made herein are subject to the penalties of 18 Pa.C.S.
Section 4904, rclatinl to unsworn falsification to authorities.
