1. The Plaintiff Dauphin Deposit Bank and Trust Company
("Dauphin Deposit"), is a Pennsylvania Corporation licensed under
the banking laws of the Commonwealth of Pennsylvania with an office
located at 3607 Derry Street, Harrisburg, Dauphin County,
Pennsylvania.
2. The Defendant, Patricia M. Kawl1ch, also known as
patricia Kawl1ch Kyle, is an individual and was last known to
reside at 121 Cold Springs Road, carlisle, PA 17013,
3. On or about December 16, 19BB, the Defendant executed a
"Consuaer Loan Note" in favor of Dauphin Deposit for the purpose of
obtaining a loan in the amount of $3,286.0B (Exhibit "A").
4. Pursuant to Exhibit "A" Defendant agreed to make pay1lent.
in 24 aonthly installments of $136.92 to Dauphin Deposit.
