AND NOW, this 21st day of November, 1994, comes the Appellant, George E. MilIovich,
by and through his allomey, Gary Lysaght, Esquire, and respectfully appeals the suspension of his
driving privileges.
1. The appellant, George E. MilIovich, Pennsylvania Operator's Number 13444122,
resides at 98 Regency Woods North, Carlisle, Cumberland County, Pennsylvania 17013.
2. The appellant received a notice of suspension dated October 18, 1994 for failing to
satisfY a judgment levied against him, Court #: 1460 S Term: 1992, Said notice is marked Exhibit
"A" and incorporated herein by reference.
3. The aforesaid suspension/revocation notice will be for an indefinite period effective
November 29, 1994.
THE AUTOMOBILE ACCIDENT
4, Paragraphs I through 3 are incorporated herein and made a part hereto.
S. The judgment against the appellant was levied as the result of a motor vehicle accident
from 1992, Hannon v. MilIovich. Docket #1460 S 1992. When the Plaintiffs in that action
brought suit, Mr, MilIovich was not in the physical or mental condition to defend himself. A
doctor's note describing Mr. MilIovich's conditions is marked Exhibit "B" and incorporated herein
by reference.
5. Mr. MilIovich currently uses his vehicle to travel back and forth to therapy several

Your driving privilege will not be restored until you
prOVide proof to the Department that all motor vehicles reg-
istered in your name are covered by a motor vehicle liability
insurance policy or by a program of self-insurance approved
by the Department end an official certificate of satisfaction
from the Prothonotary of DAUPHIN County.
This insurance information will not be accepted more
than one (1) month before the eligibility date of the resto-
ration of your driving privilege.
In order to complY with this sanction you are required
tG - return any' 'cur'rent - driver's license, learner' s permit
and/or temporary driver's license (camera card) in your pos-
session no later than the effective date listed. If you
cennot complY with the requirements stated above, you are
requirad to submit a DL16LC Form or a letter acknowledging
the sanction of your driving privilege. Failure to complY
with this notice shall result in this Bureau referring this
matter to the Pennsylvania state police for prosecution undar
SECTION 1571(a)(4) of the Vehicle Code.
When the department receive3 your license or acknowl-
edgement, it will send you a receipt. If you do not receive
this receipt within 15 deys contact the department imme-
diately. Otherwise, you may not be given credit toward serv-
ing this sanction.
