2. James W. Sauve Jr. is an individual residing at 282 st. John
Dr., Camp Hill, Pennsylvania 17011, and is hereinafter referred to as
"Defendant."
3. Defendant received a charge card account and agreed to be bound
by the terms and conditions therof.
4. Defendant received a statement of Account (hereinafter
"Statement") which set forth all debts and credits on the account for
each month.
5. Attached hereto and made part hereof and marked Exhibit "A" is
a copy of statement which evidences the debt of Defendant.
6. Defendant defaulted for failing to make payment when and as
due, and Plaintiff incurred a loss.
7. The outstanding balance due from Defendant to Plaintiff is
$9,680.69, plus interest from December 14, 1993 at the contract rate.
8. Demand for payment has been made upon Defendant, but Defendant
has failed or refused to pay.
9. Under the provisions of the charge card account, Plaintiff is
entitled to reasonable attorney's fees of 20.000% of the outstanding
balance due or $1,936.13.
