Title Certificates and records of purchases of any vehicle,
rolling stock, boats, trailers, or other items of personal property
having a title certificate in which the subpoenaed party hereto has
any interest in whole or in part, or is an owner or co-owner, along
with the current bank statement or statements, together with
savings, passbooks or checking book, currently in use for all bank
accounts in which the subpoenaed part is an owner or co-owner,
whether savings, checking or other accounts, for any account of any
nature for which the subpoenaed party is wholly or jointly
interested or has a priority or main interest.
The original certificate or certificates of deposit evidenoing
ownership in any certificate of deposit or fund in which the
subpoenaed party has an interest either jointly or individually.
Certificates of stocks or bonds in which the subpoenaed party
is jointly or individually an owner.
A current financial statement of the subpoenaed party.
Tax Return or copy thereof for the current or most recent year
filed with the United states Government and the Commonwealth of
Pennsylvania.
A list of all claims in which the subpoenaed party has against
others, including a list of all account receivables or any business
in which the subpoenaed party has a priority or ownership interest.
The original or copies of all deeds, agreements or other
similar documents, whether recorded or unrecorded evidencing any
interest, equitable or legal, in any real estate in the
Commonwealth of Pennsylvania or elsewhere, in which the subpoenaed
party has any interest, either joint or individual, whether said
interest is a present or future interest.
All documents in the possession of the subpoenaed party
relating to any Estate, Trust or similar fund or future fund in
which the subpoenaed party has any interest, as a beneficiary,
devisee, legatee or in which the subp~enaed party is an heir.
A list of all obligations owned by the subpoenaed party
whether secured or unsecured, including evidence and information.
and information available relative to such security as has been
given to the subpoenaed party to secure such indebtedness to the
subpoenaed party.
All documents in the possession of the subpoenaed party
relating to any personal or real property the subpoenaed party has

leased by Hampden Hearth, Inc. on November 3, 1988 a copy of said
Lease Guarantee is attached hereto marked Exhibit "A" and
incorporated by reference herein.
4. The Plaintiff maintained books of account, keeping an
accurate running account of all debits and credits for the lease of
said equipment provided to the Defendant, and said lease is in
default and payments theron due from lessee who has not paid
plaintiff.
5. As a result of the foregoing the Defendant owes the
Plaintiff the amount of $10,563.25 plus interest at the legal rate
of 6 percent per annum, for said lease equipment and statement of
account showing said balance on said account is attached hereto,
marked Exhibit "B" and incorporated herein by reference.
6. Demand for payment on this account has been made on
August 9, 1994. In spite of this demand, the Defendant has refused
to pay said balance of $10,563.25 plus interest at the legal rate
of 6 percent per annum, plus 20 percent attorney's fees.
1fIIBREFORB, Plaintiff demands judgment against the Defendant in
the sum of $10,563.25 plus interest from January 14, 1994 at the
rate of 6 percent per annum, plus 20 percent attorney's fees and
