' , .; 1994 fOllowing the receipt of a complaint that the Respondent had become extremely hostile to the apartment management and made the following observations regarding her: A, She was dressed in yellow lightweight pajama bottoms and a white, patched, long sleeve blouse that were soiled; 8, Her personal hygiene was poor; and C, She appeared malnourished and dehydrated. 5. During the visit on Deoenlber 2, 1994, the Petitioner's authorized representative found the Respondent in bed, stairing at the ceiling, and unaware of the caseworker's presence until touched on the arm. 6. The Respondent advised the Petitioner's authorized representative on December 2, 1994 that she had no recollection of the incident with management and claimed that management stole a book; but, when offered assistance, by Petitioner's caseworker in getting the book back, the Respondent stated that she would jump out the window if this was done. 7. During the visit on December 2, 1994, the Respondent was: A. Teary eyed during most of the visit; 8. Disoriented as to place and time; c. Under the impression that she was living in Philadelphia; and D, Speaking of her brother as though he was alive. 8. Numeroue notes written by Respondsnt were tound laying around her apartment, which notes wsre disjointed in nature and reflected Respondent's beliet that people were stealing thinqs trom her ISpartmsnt. 9. That assistance was ottered durinq the visit of Dscember 2, 1994 to qet Respondent another apartment and in takinq her to see a medical doctor, but such assistance was refused. 10. That, on or about December 7, 1994, Petitioner received a report indicating that eviction proceedings had been initiated aqainst the Respondent because she had refused to pay her December rent. 11. That, as a result of the report received on December 7, 1994, Petitioner's authorized representative contacted the Respondent regarding the eviction proceeding and was informed by Respondent that she did not have to pay rent because she had moved out of her apartment and, later, claimed that she was exempt from paying rent because she was over eiqhty (BO) years of age. 12. That, on December B, 1994, Petitioner's authorized representative visited the Respondent at her apartment and, during this time, Respondent appeared aloof, was aimlessly walking throuqh her apartment, and when asked about her rent payment, claimed to hav. paid three months in advance and that management had stolen the receipt. 13. That Petitioner has been providing Respondent weekly grocery shopping and meal preparation two (2) times a week. 14. That during the month ot Decamber, 1994, Respondent has become increasingly resistant to services being offered in that: A. Groceries were last purchased on December 2, 1994; and B, The last day she permitted meal preparation was December 8, 1994; 15, That, on December 9, 1994, she was given tood by the apartment management which she discarded uneaten, 16. That, since Deoember 9, 1994, Respondent appears to have consumed only some cheese, a few slices ot lunch meat and a can ot soup other than meals that were provided by Petitioner. 17. That home care aides are refusing to return to Respondent's apartment because of tear that she may become violent. 18. That, during the visit of Petitioner's authorized representative on December 19, 1994, Respondent was extremely agitated, easily provoked and irrational, 19. That, on September 30, 1994, the Respondent WGS evaluated by Dr, David Petkash, a psychiatrist at Holy Spirit Mental Health Center, with ISn initial diagnosis of senile dementia, alzheimer's, type, with probable accompanying delusional features, 20, That, in order to rule out any msdical problems that could contribute to her physical and mental conditions, Dr, petkash ordered further laboratory testing which Respondent refused, 21. That Respondent tailed to attend another session with Dr. David Petkash on October 31, 1994, 22. That an inpatient geriatric assessment be performed all soon as a bed is available at the Hershey Medical Center which is the least restrictive, involuntary protective services that is appropriate and would remedy the situation. 23. That a geriatric assessment would remove the condition which presents an imminent risk of death or serious physical harm to the Respondent by making a determination of her physical and mental condition. 24.
