AFFIDAVIT. OF CONSENT TO DIVORCJ,
(1) A Complaint in divorce under Section 3301 (c) of the
Divorce Code was filed on Janullry 26/ 1995.
(2) Defendllnt acknowledges that service of the Complaint
was maete by Acceptance of Service by Johnnll J. Deily I Esquire
on February 6, 1995.
(3) The marriage of Plaintiff
irretrievllbly broken and ninety dllYS have
of filing the Complaint.
(4) I understand that I may tose rights concerning
lllimony, division of property / lawyer's fees or expenses if I
do not claim them before II divorce is granted.
