Procedure 1915.15 for the proper form and contents of a custody
complaint in Pennsylvania, and it is requested that such a
complaint be filed and served upon the proper parties prior to the
hearing.

STIPULATION FOR MODIFICATION OF CUSTODY,
WHEREAS, the parties are the parent and relative of the minor
child, DONNA and
WHEREAS Donna is in the sole legal custody of the plaintiff; and
WHEREAS Donna is residing with the defendant; and
WHEREAS the parties agree that it is in the best interests of the
child that she remain in the care and custody of the plaintiff;
Now therefore the parties hereto hereby stipulate and agree that
the judgement of October 23, 1993, should be further modified with
regard to custody as follows:
CUSTODY The plaintiff shall have sole custody of the minor child
of the parties (Donna Lee).

To take and maintain custody of my child. DONNA L. BRADLEY: to do all
acts necessary or desirable for maintaining her health, education, and
welfare, Including the registration and enrollment of my child In
educational programs and schools: and to maintain her customary ,living
standards, including. but not limited to. provision of living quarters,
food, clothing, medical, surgical and dental care, entertainment and
other customary matters: and, specifically, to approve and authorize any
and all medical treatment deemed necessary by a duly licensed physician
and to execute any consent, release or waiver of liability required by
medical or dental authorities Incident to the provision of medical,
surgical or dental care to her by qualified medical personnel; to act in
loco parentis to my child, DONNA L, BRADLEY.
