Dear
Erie Insurance Exchange has agreed to accept payments for
damages incurred in the accident of 1./-19 , 19 9"
Subject to the following terms and conditions:
1. PAYMENTS. You will pay Erie Insurance EJsphange the sum
of $ .3CJOO ~ , payable as follows: $ ~~ every
month,beginning A1.<fH?-I'-u I, 19 9'::' . You shall make
your payments in this manner until the total amount due is paid in
full.
2. PREPAYMENTS. You may also make payments ahead of
schedule, however such payments will be applied at the end of
their payment schedule. In other words, a prepayment will not
relieve you of your Obligation to make a regular payment the next
month fOllowing the month in which you make a payment.
3. ENTRY OF JUDGMENT. Although you agree to make payments
to Erie, you will execute contemporaneously herewith a JUdgment
Note in favor of Erie Insurance exChange for the full amount due
hereunder and we willJ=equest that the Prothonotary of the Court
of Common Pleas of ~~K.L/Ai County, Pennsylvania
enter jUdgment against you for the full amount of the JUdgment
Note.
4. ENFORCEMENT OF JUDGMENT. Erie Insurance Exchange will
not attempt to certify it's jUdgment to the Pennsylvania
Department of Transportation for the purpose of having your
driVing privileges revoked if each payment is made on or before
the date it is due. However, if you fail to make any payment on
time, Erie Insurance Exchange may take the necessary steps to have
~our driVing privileges revoked.
