Date of Contract: February 24, 1995
Premises:
Contractor, for the benefit of Owner, hereby represents and agrees as follows:
1. That the date of the last labor performed or materials furnished with
reference to the above-referenced Project and Contract was March 6, 1995;
2. That Contractor has been paid in full by Owner and that Contractor has
paid in full all of its subcontractors and suppliers with regard to the Project;
3. That Contractor hereby releases any rights which it has or may have to
file a mechanic's lien against the Premises;
4. In the event that it is necessary for Contractor or any subcontractor or
material supplier of Contractor to perform addillonal work with regard to the Project,
whether in the nature of furnishing missing items, repairing defecllve workmanship or

materials, or replacing defective workmanship or materials, Contractor hereby waivers

any right to file a mechanic's lien against the Premises, on behalf of Itself and on behalf
of Its subcontractors and material suppliers, on account of any such additional work or
materials; and Contractor agrees that any subcontractor or material supplier furnishing
labor or materials with regard to the Project after the date hereof will be given prior
notice that its mechanics' lien rights have been waived;
5. That Contractor will Indemnify Owner and hold Owner harmless with
regard to any mechanic's lien flied against the Premises by Contractor or by any
subcontractor material supplier of Contractor.
IN WITNESS WHEREOF, Contractor, intending to be legally bound hereby, has
