4. Petitioner desires to change his name from Robert H. Reetlng, II, to Nlc6 KaI Mancini, due
to his lack of afT'iliation with Ws present name, Petitioner has requested this Honorable Court to change
Ws name from Robert Henry to Nlc6 KaI, the name of Ws maternal great, great grandfather.
6. Petitioner does not desire to change Ws name due to the fact that he has no afT'illstion with
his natural father, but desires to change Ws name due to the fset that he believes he has no Individual
