2, The Defendant is Deborah C, Holloway, an individual with
a place of residence located at 17 South Queen Street
Shippensburg, PA 17257,
3, At the written request of the Defendant, Plaintiff issued
to Defendant a VISA credit card, account number 4428002517401024
and at all times relevant hereto, Defendant was the holder of
said card issued through Plaintiff's credit facilities, A true
and correc~ copy of Defendant's application is attached hereto,
made a part hereof and marked Exhibit "A",
4, Under the terms and conditions of Exhibit "A", Defendant,
upon acceptance of Plaintiff's credit card, agreed to be bound by
the terms and conditions of plaintiff's revolving credit plan
agreement, A true and correct copy of said agreement is attached
hereto, made a part hereof and marked Exhibit "B".
5. The Defendant subsequently received a cash advance from
Plaintiff and incurred additional obligations in the sum of

Catherine H, Pickhover states that she is the Operations Officer of First Deposit National
Bank and is authorized to take this affidavit on its behalf and that the facts set forth in the
foregoing complaint are true and correct to the best of her information, knowledge and belief.
She further states the annexed account against DEBORAH C. HOLLOW A Y is correctly copied
from the books of original entry of First Deposit National Bank that the charges were made in
said books at or about the time of their respective dates; that the charges are correct and the
account is just and true as stated; that there is now due and owing thereon the sum of $5274,45
plus interest and attorney fees; that no part of said sum has been paid or in any manner settled;
and that there are no deductions or offsets of any kind, except as are therein specified and
credited, She further understands that false statements made herein are subject to the penalties
of 18 Pa.C,S, Section 4904, relating to unsworn falsification to authorities,
