1. The parties to thls action separated on April I, 1993 and have continued to live separate and
apart for a period of at least two years.
2. The marriage is irretrievably broken.
3. I understand that I may lose rights concerning alimony, division of marital property,lawyer's
fees or expenses if I do not claim them before a divorce is granted.

ORDER OF COURT
AND NOW, this ~ 1[, day of June, 1995, upon consideration of
the Praecipe To Transmit Record in the above-captioned matter, and
it appearing that the praecipe does not contain the information
required under Pa. R.C.P. 1920.73 respecting the date of execution
of plaintiff's affidavit required by Section 3301(d) of the Divorce
Code and the date of service of plaintiff's affidavit upon
Defendant, and that the record does not include a proof of service
of plaintiff's affidavit as required by Pa. R.C.P. 1920.4, 412(a),
403, 405, a divorce decree will not be entered at this time,
without prejudice to Plaintiff's right to supplement the record and
file a revised praecipe. Plaintiff is also requested to file a
copy of the county-affidavit sent to Defendant with the notice of
intention.
