3. On October 16, 1991, the Defendant mede, executed and delivered to
the Plaintiff a Loanliner Application, a copy of which Is attached hereto as Exhibit "A"
and made a part hereof.
4. Defendant submitted his loan request to Plaintiff at Plaintiff's place of
business, and same was approved therein, and subsequently the Defendant having
delivered the said Loanliner Application to the Plaintiff and making withdrawals
thereon, the Defendant became liable for the payment of said loan in accordance with
the terms and conditions thereof.
6. On June 24, 1993, the Defendant made, executed and delivered to the
Plaintiff a Note (as an addendum to his Loanliner Credit Agreement dated October 16,
1991), In the total amount of ELEVEN THOUSAND TWO HUNDRED TWENTY-TWO
DOLLARS AND SIXTY-FOUR CENTS ($11,222.64). a copy of which is attached
hereto as Exhibit "B" and made a part hereof.
6. Defendant submitted his loan request to Plaintiff at Plaintiff's place of
business, and same was approved therein, and subsequently the Defendant having
delivered the said Note to the Plaintiff, the Defendant became liable for the payment
of said Note In accordance with the terms thereof.

5. The Defendant has failed to make regular payments on said Note and has
since become In default of the payment of said Note and the entire balance according
to the terms and conditions thereof Is due.
6. The outstanding balance as of March 15, 1995, on said Note Is NINE
THOUSAND SEVEN HUNDRED SEVENTEEN DOLLARS AND SIXTY-ONE CENTS
($9,717.61), as shown on Plaintiff's Statement of Account, attached hereto as
Exhibit "C" and made a part hereof.
7. Due to Defendant's default In payments, interest has been added in the
amount of SEVEN HUNDRED SEVEN DOLLARS AND FORTY-TWO CENTS ($707.42)
to which Defendant Is additionally liable.
8. In accordance with the terms and conditions of said Note, Defendant
promised to pay to the Plaintiff reasonable attorneys' fees and, accordingly, now is
additionally Indebted to the Plaintiff in the amount of TWO THOUSAND FIFTY-SIX
DOLLARS AND THIRTY CENTS ($2,056.30).
9. Plaintiff has frequently demanded payment from Defendant of said
amount due and owing as aforesaid, but Defendant has refused and neglected and stili
refuses and neglects to pay said amount or any part thereof.

WHEREFORE, Plaintiff brings this suit to recover from Defendant the sum of
TWELVE THOUSAND FOUR HUNDRED EIGHTY-ONE DOLLARS AND THIRTY-THREE
CENTS ($12,481.33), together with interest thereon at a rate of twelve and nine
tenths percent (12.9%) from March 15,1995.
