2. Hespondent, Pennsylvania Department of Transportation, Bureau of Driver's
licensing, has a mailing address of IIJ3 Transportation allll Safety Building, Ifarrlshurg,
Dauphin County, Pennsylvania.
3. Your Petitioner received a Notice of I.icense Suspension wllh a Illail date of

cannot comply with the requirements stated above, you ara
required to submit a DL16LC Form Dr a letter acknowledging
the sanction of your driving privilege. Failure to complY
with this notice shall result in this 8ureau referring this
..atter to the Pennsylvania state police for prosecution
under SECTION 1571(a)(4) of the Vehicle Code.
Credit will not begin until all current driver's
license product(s), the DL16LC Form. Dr a letter
acknowledging your sanction is received in this Bureau.
When tha department recaives your license Dr
acknowledgement, it will send you 8 receipt. If you do not
receive this receipt within 15 days contact the department
immediately. Otherwise, you may not be given credit toward
serving this sanction.

of the county of your residence within 30 days of the mail
date (APRIL 26, 1995) of this notice. Sending a copy to
this departlllent of a t1lllely, filed appeal will stay tha
