Plaintiff. by and through his attorneys. Morgan & Morgan, I',C,. hereby
complains against Defendanl and avers as till1ows:
1. Plaintiff is an adult individual residing at the above address,
2, Defendant is an adult individual residing at the above address,
3. On or about November 23. 1993. Plaintiff was traveling Nonh on State
Road 2004. when Defendant. operating a vehicle South on State Road 2004 entered his lane
of travel and caused a collision, As a result of thatuccldent. Plaintiff suffered severe and
disabling injuries, as arc Illore fully set forth below,
4. The said accident was due tu the negligence, carelessneNS and recklessness
of Defendant, including:
A. Fuiling to keep his vehicle under prnller l'ontrnl;
II, Falling III keep u proper lookout;
C. Entering opposing lane of traffic;
D, Crossing the center line 01 the roadway;
E. Fuillng III wuw I'luillliff of un unreusonuble risk of harm;
F, Traveling ut an unsafe speed;
G. Striking Plaintiff's vehicle;
II. Violllting state IInd local lilli'S and ordinllnces rellltive

5, As a result of the IIccldent. Plaintiff suffered severe and disabling Injuries,
including but not limited to, injuries to his nerves, bones. muscles, joillls and fascia. pain
and suffering, mental and emotionlll distress, which arc or may be continuing in nature,
ll, As II further result of his injuries. I'lamtiff incurred medical bills IInd
expenses, lost wages and/or earning capacity and has suffered diminution of his dally
activities and loss of enjoymelll of Iife's pleasures, which arc or may be continuing In nalure,
7, As a further result of the accident, Plaillliff suffered damage to his vehicle.
WIIEREFORE, Plainliff demands judgment against Defendant in an amounl in
excess of $20,000,00. exclusive of interest. costs and delay damages.

Plaintiffs COll1plaint, and 10 enter judgment against the Plaintiff and in favor of the
Defendant.
